Unconscious (Rules)

Rulebook

When you’re unconscious, you can’t act. You take a –4 status penalty to AC, Perception, and Reflex saves, and you have the blinded and off-guard conditions. You fall prone and drop held items.

At 0 HP (Not Dying)

If you’re unconscious at 0 HP but not dying (e.g. stabilized), you naturally return to 1 HP after at least 10 minutes (GM determines exact time). If you receive healing, you wake immediately.

At More Than 0 HP

If knocked unconscious while above 0 HP (sleep, effect, etc.), you wake up if:

  • You take damage (unless it drops you to 0 HP)
  • You receive healing
  • Someone shakes you awake (Interact action)
  • Loud noise nearby — at the start of your turn, attempt a Perception check vs. the noise’s DC (DC 5 for a battle, or the creatures’ Stealth DCs if they’re being quiet)

Some magical sleep effects don’t allow the Perception check to wake up.
